Environment Evaluation
Aging in place requires a safe home. We provide an evaluation of the environment to optimize safety in order for daily living to continue at home.
RN
Assessment
An assessment can be performed to help determine current physical (heart/lungs), cognitive and emotional health. This may also be collaborative to include medications and the goals of care. Communication with your healthcare providers as needed.
Coordination
of Care
Today's healthcare systems is complex and requires diligent oversight. Communication with healthcare providers can improve outcomes. For out-of-town families, we can be available to guide you through unexpected events.
